Errata in

We overquoted the literature (papers by Mella and Ballico). 

Theorem 2.3 is true for d\ge 4, but for d=3 [Chiantini-Ottaviani-Vannieuwenhoven] found 
an additional exceptional case, which is rank 9 in S^3C^6, where there are exactly two decompositions.
